Premises Identification Survey

As a part of its ongoing efforts to safeguard U.S. animal health, the U.S. Department of Agriculture (USDA) initiated the implementation of a National Animal Identification System (NAIS) in 2004. The NAIS is a partnership of State and Federal animal health officials and is being established as a phased-in approach of three components: premises identification, animal identification, and animal tracking. In the survey below, please truthfully respond to the following questions about NAIS and the Wyoming registration process.

You may either automatically submit your responses from the Internet or print a copy of the survey and mail it to the Wyoming Livestock Board. The survey will only take between 2 to 5 minutes.
