STEP 2 – Print-out the appropriate application form (either the Livestock Premises Application or Non-Producer’s Premises Application). STEP 3 – The first two pages of both applications will require you to fill in primary owner’s contact information, property’s legal description, type of business, species contained within premises, and secondary contact information. The non-producer’s form has additional information pertaining to the facility type and legal description on the third page. The producer application consists of two pages while the non-producer form is three pages. STEP 4 – After completing the appropriate form, mail the application to address listed on the second page. Be sure that you have filled in all the required information. Required fields are indicated with an asterisk mark placed beside the field. Please allot four to eight weeks for a delivery of a premises identification number. If you any questions regarding premises identification call (307) 777-5941. |
